| Progesterone is a steroid produced by the adrenal cortex, the corpus luteum, the testes and the placenta. As with other steroids, progesterone is synthesized from cholesterol via a series of enzyme-mediated steps [1,2]. Progesterone produced by the adrenal cortex and testes is almost immediately shunted into other steroidogenic pathways, with only a small amount released into the blood. Progesterone produced by the corpus luteum, and during pregnancy by the placenta, serves several independent physiologic functions related to its ability to convert estrogen-primed uterine endometrium into secretory endometrium [3]. These functions include preparation of the uterine endometrium for implantation of the fertilized oocyte and maintenance of the endometrium during pregnancy. Progesterone has multiple other actions in female reproductive physiology, including participation in the feedback regulation of pituitary gonadotropins and ovarian estrogens, stimulation of mammary gland maturation during lactation, maintenance of the vaginal epithelium, uterine smooth muscle relaxation during embryogenesis, and stimulation of basal body temperature. Serum progesterone levels are relatively high in women during the luteal phase of the menstrual cycle and during pregnancy due to gonadotropin stimulation. Measurement of serum progesterone concentrations have been used in evaluating ovarian function [3,4].
