Insulin and C-peptide determinations are used for the differential diagnosis of hypoglycemia. Elevated levels raise the possibility of an insulin-producing tumor (insulinoma). C-peptide measurements may be used to distinguish between these two possibilities. Commercial insulin preparations do not contain C-peptide, which is therefore expected to be low in cases of exogenous insulin administration. In contrast, both C-peptide and insulin are released in equimolar amounts and both are expected to be elevated in cases of insulinoma.
