| Erythropoietin (EPO) is the primary known naturally-occurring stimulant of red blood cell production. The predicted molecular weight of human EPO is 18.2 kDa. Due to N-glycosylation, the circulating form of EPO has an estimated molecular mass of ~30.5 kDa. Although EPO itself appears to have a unique structure, the EPO receptor bears some homology to several other growth factor receptors, including those for interleukins 2 and 6, prolactin and growth hormone, suggesting possible functional similarities for these growth factor systems [1].
In the fetus, the liver is the major site of EPO production [1, 2, 3]. Postnatal production occurs primarily in the renal tubular epithelium, although significant continued postnatal hepatic production has been demonstrated [4]. EPO is secreted into the circulation from where it is taken up by target tissues. EPO binds to specific cell membrane receptors on erythroid progenitor cells and appears to facilitate erythrocyte differentiation, perhaps by increasing survival of the progenitor cells. Endogenous, N-glycosylated EPO is cleared at a constant rate, with a half-life estimated at 1.5 - 2 hours. The clearance rate is largely unaffected by EPO levels or tissue utilization. Therefore, circulating EPO levels reflect production rates [5].
