| Parathyroid hormone (PTH) is synthesized by the parathyroid glands [1,2] and secreted in response to decreased extracellular calcium concentrations. PTH prevents hypocalcemia by enhancing bone resorption and decreasing calcium excretion by the kidney. Following release into the circulation, intact PTH is rapidly degraded into 4 fragments: N-terminal (residues 1-34), C-terminal (residues 35-84) and 2 mid molecule fragments (residues 44-68 and 35-64). The N-terminal fragment retains full biological potency, and is also rapidly degraded in the blood. The biologically inactive C-terminal and mid-molecule fragments are present in the circulation for a much longer period of time, resulting in C-terminal and Mid-Molecule concentrations which are approximately 5 to 10-fold higher than Intact or N-terminal PTH concentrations.
PTH acts directly on bone and kidney and indirectly on the intestine to alter calcium and phosphate concentrations in serum and urine. PTH is helpful in discriminating between primary hyperparathyroidism, where PTH levels are elevated, and nonparathyroid hypercalcemia [3,4]. Additionally, PTH is helpful for assessing parathyroid function in renal failure and is often used for dialysis patients.
